Forsyth Park Medical Centre is a purpose built medical centre designed for the comfort and ease of access for patients. There are no ramps or stairs and the front door is automatic. This makes the practice easily accessible for prams, the disabled and the elderly. Toilet facilities are provided for the disabled. We have a large car park with approx. 53 spaces including 2 disabled car parks.
We have two well-equipped treatment rooms for minor procedures and emergencies. Our waiting room is large with multiple televisions displaying patient information and entertainment, it also includes a children’s play area. A wide range of medical educational pamphlets are available for patients. Topics covered are cholesterol, diabetes, breast self-exams, pap smear, asthma and many more.

Fees and Billing ArrangementPatient under the age of 16, pension card holders, healthcare card holders and DVA card holders will the fully bulk billed. Patients that don't meet this criteria will be required to pay for their consults which will results in a small out of pocket cost. Please see our website for full fee breakdown.

Workcover and TAC PatientsInitial Workcover and TAC consults must be paid at the time of consultation as these are not covered by Medicare. You will then be given a receipt to claim from your employer/TAC. If you already have a claim number and insurer the invoices will be sent directly to them

Out of Hours ArrangementsFor medical attention out of the clinic’s opening hours, home visits and free medical phone advice can be obtained by calling the locum service on 132 660

Phone CallsDoctors are unable to take phone calls from patients while consulting unless the matter is urgent. Reception staff will take a message and the call will be returned when during the Doctor’s next available break.

PrivacyPersonal information is collected only when necessary to provide patients with a quality health service and as required by HIC, Australia. Personal information will only be disclosed to other parties (eg. Other health professionals) with permission of the patient. ComplaintsIf you have any concerns about any aspect of the service provided, please address a written complaint to the practice manager and appropriate steps will be taken to resolve the problem immediately or you can contact Health Complaints Agency on 1800 136 066. For Feedback or comments we also offer a suggestion box at reception.
